Co-payments are set according to a discounted schedule which is based on family income and approved by the Southwest Center Authority Board. No one will be denied services because of inability to pay.

The Center is the Prepaid Mental Health Provider for all Medicaid enrollees in the designated counties of Southwest Utah.
If you live in Beaver, Garfield, Kane, Iron or Washington counties; have a Medicaid card and need mental health services (psychiatric, hospital or clinic), Southwest Center is your provider. All of your mental health care must be approved by Southwest Center before you or any family member can receive services to be paid by Medicaid.
Contact the office nearest you and request services. The office in your area will evaluate the need for services and establish a plan with you. This can be done simply by calling your local office or our toll free 1-800-574-6763 number, available on a 24-hour basis. Any primary physician or other provider MUST call Southwest Center for pre-authorization for placement in any special program other than those offered by Southwest Center.

Southwest Center is a political subdivision of the State, created by means of an Inter-County Cooperative Agreement between the counties of Beaver, Garfield, Iron, Kane and Washington. As such, the Center is supported by funding from the five counties, the Utah State Division of Substance Abuse, and the Utah State Division of Mental Health. Southwest Center is administered by an Authority Board comprised of county commissioners from each county.

In accordance with provisions of Utah State law (UCA 26-17-6), the elected county commissions from Beaver, Garfield, Iron, Kane and Washington counties have established a mental health/alcohol and drug authority board, comprised of county commissioners from each of these counties, to govern the operation of the Southwest Center.
